Returning patients:  You will need to be at your appointment 15 minutes early to learn our new check-in procedure. (And to browse our new lobby!) 

All Patients (New and Returning):  Please fill out the Patient Information Sheet and the Patient Intake Form and submit forms to us a minimum 24 hours before your appointment.  NLC staff needs time to enter the information into our new electronic system before you can be seen by Dr. Orwick.  Please read through the consent forms and be prepared to electronically sign them in the office. 

In the future, check-in is going to be such a breeze that we know you won’t mind a little work to prepare!
